What dose a home health nurse do? 

A home health nurse provides one-on-one care tailored to your needs. They coordinate with your doctor to develop and manage your care plan so you can focus on recovering and being independent once again.

 

 

 

 

From basic medical care to more complex treatments, our skilled nurses may help you by:

 

 

 

 

 

  • Performing head-to-toe physical assessments

 

 

  • Checking vital signs such as heart rate and blood pressure

 

 

  • Administering injection and infusion therapy, or managing a catheter

 

 

  • Providing wound care such as dressing changes and monitoring complex, non-healing wounds to make sure they heal properly without infections

 

 

  • Assisting with the maintenance and care of ostomies, including education on how to properly clean and change them

 

 

  • Explaining your condition or diagnosis, common symptoms and how those symptoms may affect you

 

 

  • Educating you on your medicine and how to manage it, including safety tips and possible drug reactions and side effects

 

 

  • Helping to improve healthy eating by teaching you about nutrient-rich foods and creating a meal plan to fit your needs and health conditions

Depending on your needs and doctor’s orders, you may receive care from additional types of home health clinicians like physical therapists, occupational therapists, speech-language pathologists or medical social workers.
